Potomac Case Management and Related Services values a positive work environment, providing a comprehensive single point of entry to community services in Washington County, with respect and results that empower our consumers to reach their greatest level of autonomy based on their individual abilities.

Each of our program pages also offer a downloadable referral form so that you can help us help those in the community. Please fill in all requested information on the form so that we may be able to determine their eligibility as quickly as possible. All referrals can be faxed to our office.

However, the original referral must  be mailed to our office as well.
