General Case Management

General Case Management is a service to assist persons with a mental health diagnosis to build safe bridges with the community.

Potomac Case Management utilizes a team based model. In this model the consumer is the integral member of the team. He /she will be encouraged, supported and expected to participate in and approve every decision of their service plan. Each consumer is assigned a case manager but will recognize a broader level of support with backup support from the case management team as well.

 

 

What Services Are Offered? 

Each person and their situation is unique. Each person will assist in completing their own assessment for which a service plan is jointly developed. Each plan is different and could include:

-Initial assessment

-Re-assessment as progress is made

-Service planning

-Case management

-Service linkage

-Monitoring of service delivery

-Consumer advocacy

-Crisis intervention

-Case coordination

-Family support

-Consumer outreach

-Schools

-Mental Health Providers

-All service agencies

 

How to make a referral

If you wish to make a referral, please feel free to download our TCM REFERRAL. We urge you to also download the GUIDELINES FOR ADULT REFERRALS and the GUIDELINES FOR CHILD AND ADOLESCENT REFERRALS. Please fill out all sections completely. Once the referral is completed, you may fax it to our office. However, the original referral must be mailed to us as well. Please Do Not E-mail referrals due to confidentiality issues.
